Aligarh: Professor Jamal Ahmad, Director of Rajiv Gandhi Centre for Diabetes and Endocrinology, Jawaharlal Nehru Medical College, Aligarh Muslim University has urged diabetes patients to take special health precautions during the holy month of Ramadan.

Quoting the religious scripture, which exempts the sick from the obligation of fasting, Prof Jamal said that a diabetic patient must immediately break the fast if blood glucose is less than 70 mg/dl or it exceeds 300 mg/dl by glucometer.

“In case blood glucose drop to 70mg/dl in early hours of the fast, particularly if Sulfonyl Urea or Insulin have been taken at pre-dawn (Sahar), the fast should be terminated,” said Prof Jamal.Ramadan1

Warning the Type 1 Diabetes patients taking four Insulin injections per day to avoid fasting, Dr Jamal said that on fasting these patients are at a risk of getting into diabetic coma.

“However, patients with Type 2 diabetes can fast without any health risks provided they take Metformin ones soon after the sun-set meals during Ramadan.

He further advised the diabetic patients on Metformin and Sulfonylurea, observing fasts to continue the Metformin in the usual dose and consuming Sulfonylurea tablet before dinner.

“Those consuming Sulfonylurea and are on stable Glycemic Control, should consume Sulfonylurea from the usual morning dose to before the sunset meal,” said Prof Jamal who added that those on twice a day Sulfonylurea should have their usual evening dose with the pre-dawn meal and the usual morning dose with the sunset meal.

Prof Jamal also advised patients on Metformin and DPP4 and having Glycemic Control to continue their evening dose and reduce the morning dose to half.

“A diabetic patient should arrange a pre-Ramadan fasting consultation with a doctor or diabetes educator to review control and feasibility of fasting safely,” pointed Prof Jamal. He further urged diabetic patients to record weight daily and inform their doctors if their weight changes more than two kilograms.

During the Ramadan fasts, it is important to learn the warning symptoms of hypoglycaemia and hyperglycemia, said Prof Jamal.

He also warned diabetic patients against over eating while breaking their fasts.
